Plans, coordinates and provides comprehensive clinical pharmacy services. Under the direction of the Pharmacy Director and within established policies and procedures, the incumbent performs the following functions: Liaison between pharmacy and medical staff to assure accurate and consistent communication via pharmacy interventions and availability of current drug information. Monitors patient drug therapy to evaluate the appropriateness of use, dose, dosage regimen, route of administration, regimen compliance, therapeutic duplication, drug interactions and adverse drug reactions. Provides pharmacokinetic analysis on a consultative basis. Responsibility with precepting pharmacy students during their clinical clerkship. Assists with development and provision of additional clinical services. Assists in publication, distribution and maintenance of Drug Formulary. Reviews new requests and non-formulary requests for Pharmacy & Therapeutics Committee. Implements therapeutic substitutions and restrictions with regard to efficacy, safety, and cost-effectiveness. Conducts periodic reviews of drug formulary by therapeutic classifications. Conducts Drug Utilization Evaluations and Quality Assurance reports. Monitors Adverse Drug Reactions, prepares and reports findings to P&T Committee. Participates in discharge planning rounds and provides patient discharge medication counseling when applicable. Provides inservice education, drug information, and other educational services upon request. Reviews, prepares and presents drug information, therapeutic studies and other material for P&T review. Maintains Pharmacy Department Newsletter. Maintains currency by attending seminars, conferences, and CE programs. Responsible for development and sustainment of staff competency.

Monday through Friday days, with flexibility regarding provision of educational services upon request.
Education: Pharm D degree required Experience: One (1) year previous pharmacy experience. License/Certification: Current New York license to practice Pharmacy.
